Cooperstown, NY – Through a rigorous process conducted by the National Committee for Quality Assurance (NCQA), 33 Bassett Healthcare Network practices have sustained their Patient-Centered Medical Home (NYS PCMH) recognition through the annual reporting program.

For patients, this translates to comprehensive, coordinated, accessible, quality care focused on the whole person.

The NYS Patient-Centered Medical Home (PCMH) model is designed to address all patient care needs, delivering high-quality, cost-effective primary care. In PCMHs, a primary care doctor leads a team that partners closely with a patient throughout life. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system and has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.

The New York State Patient-Centered Medical Home (NYS PCMH) Recognition Program is an exclusive model developed in collaboration with the National Committee for Quality Assurance. It is based on NCQA’s core PCMH model which aligns with health plans and supports the state’s initiative to improve primary care and promote the Triple Aim: better health, lower costs, and better patient experience.

The mission of the National Committee for Quality Assurance (NCQA) is to improve the quality of healthcare through programs such as their PCMH Recognition program. NCQA’s Patient-Centered Medical Home Recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices (with 50,000+ clinicians) are recognized through NCQA. More than 95 organizations support NCQA Recognition.
