Bassett Treatment of Minors Policy:
Our health care practitioners believe that parents provide a most important background for their child's health and wellness, enabling us to give the best possible care to children. It is therefore our policy that a parent or parent's proxy be present whenever we treat minors, except as permitted under NYS law.
If unusual circumstances prevent you from being present when your child is seen, then we will need a signed copy of either:
- Consent by Proxy for Non-urgent Pediatric Care (authorizing someone else, who accompanies your child, to make health related decisions for you regarding your child's care)
- Pre-authorization to Treat Minors Consent (giving us a one-time OK to see your unaccompanied child)
Click on one of the links above to print a copy of the form you need, and either fax or mail the completed form to the clinic or health center where your child will be seen.
We will need the appropriate signed consent form when your child is being seen without you. These forms are also available at the clinic where your child is being seen. If you have questions about this requirement, please call 607-547-3290 or 1-800-BASSETT (1-800-227-7388).
