The New York State (NYS) Patient-Centered Medical Home program leverages health technology that supports high quality and patient-centered coordinated care. Sustaining NCQA Recognition through annual ongoing reporting demonstrates that the practice has invested in the tools, systems, and processes needed to deliver high quality and patient-centered coordinated care.
