Delaware Academy Elementary & Middle/High School-Based Health offers the following services at two separate locations; one within the elementary school, and one within the middle/high school:

The New York State (NYS) Patient-Centered Medical Home program leverages health technology that supports high quality and patient-centered coordinated care. Sustaining NCQA Recognition through annual ongoing reporting demonstrates that the practice has invested in the tools, systems, and processes needed to deliver high quality and patient-centered coordinated care.
