Bassett Healthcare Network has achieved Level 4 Medicaid Health Home status – the designation, which Bassett has maintained for six years, is critical to the health and wellbeing of some of the most vulnerable members of our community.

The New York State Department of Health first awarded Bassett Health Home status in 2014 as part of a statewide program aimed at improving the health of individuals enrolled in Medicaid who struggle with complex chronic conditions. It can be difficult to navigate the system and find the necessary providers and services, which makes it nearly impossible to get well and stay healthy.

Bassett’s Medicaid Health Home has agreements with six agencies to help more than 1,200 enrolled individuals in Chenango, Delaware, Otsego, Schoharie and Herkimer counties. The six agencies include:

  • Bassett Community Health Navigation
  • Schoharie County Mental Health
  • Catholic Charities of Chenango County
  • Catholic Charities Care Coordination
  • Southern Tier Care Coordination
  • Rehabilitation Support Service

These six agencies have more than 50 community health navigators and care managers who connect enrollees to the health care system and encourage routine visits to achieve better health and a better quality of life. Their services include:

  • Linking individuals to needed resources within the community such as transportation, housing and food insecurity services
  • Educating individuals on the management of chronic conditions
  • Educating individuals about gaps in care 
  • Organizing efforts to create a care team to address ongoing health needs

The overall goal is to improve the health of enrolled individuals, reduce unnecessary emergency department visits and inpatient hospitalizations, and help individuals achieve independence by learning to navigate community services on their own.

There are 33 designated Health Homes in New York state; sixteen serve children and 17, including Bassett’s, serve adults.
