Diabetes Education Program: Self Management Training for Diabetes

  • A comprehensive education service to assist you with diabetes self management to the best of your ability.
  • A visit with the Certified Diabetes Educator or Diabetes Nurse Educator can provide you with the tools and information needed to manage your diabetes.
  • Diabetes Education in based on The 7 Self-Care Behaviors recommended by the American Association of Diabetes Educators (AADE). These behaviors can help you with your daily diabetes care plan. The seven behaviors are Healthy Eating, Being Active, Monitoring, Taking Medications, Problem Solving, Reducing Risk, and Healthy Coping.

  • You have diabetes.
  • Your health care provider recommends you learn more about diabetes management.
  • You want to learn more about diabetes management.

  • Certified Diabetes Educators or Diabetes Nurse Educators work under the supervision of an Endocrinologist, collaborate with primary care physicians, and dietitians, for optimal management of your diabetes care.
  • Our diabetes educators will meet with you for a private consultation to personalize your care.
  • Our program follows the American Association of Diabetes Educators, also know as the AADE, recognition guidelines. The AADE Guidelines for Practice of Diabetes Self-Management Education and Training are being nationally recognized.


  • Variable clinic hours are available for individual appointments.
  • Group sessions are offered in several locations

  • As occurs with other Bassett appointments, we will submit charges to your insurance carrier. You may be responsible for a co payment or other charges, if not covered.

Family members or domestic partners that attend may do so free of charge.
