Cardiac Strength and Rehabilitation

Cardiac rehabilitation is a highly personalized treatment plan, designed to strengthen your heart muscles, and improve general fitness and stamina after a heart diagnosis or treatment. Your plan will include evaluation and monitoring, instruction on physical activity, nutrition planning, and stress management

Treatment Plans

Cardiac rehabilitation offers a wide range of cardiac strengthening activities, all designed to help reduce damage to your heart, increase stamina, and give you back the confidence to get back to the activities you love.  Cardiac rehabilitation includes:

  • Assessment of your personal risk factors for heart and blood vessel disease.
  • Education and support to make healthy lifestyle changes - including maintaining a healthy weight. heart-healthy nutrition, and avoiding tobacco.
  • Creating a personalized exercise plan that works for you.
  • Monitoring and better controlling your blood pressure, lipids/cholesterol and diabetes.
  • Stress management counseling.
  • Opportunities to meet, and share stories with other patients on a similar health journey.
  • Ensuring your doctor and other healthcare providers are all fully informed about your continuing assessments and progress following your cardiac event.


If you have one of the following diagnoses you may be eligible to participate:

Heart attack, angina, coronary artery angioplasty or stents, open heart surgery such as coronary bypass or valve surgery, heart failure or heart transplantation.

Although generally covered by health insurance companies, you should check to determine co-pays or other requirements.

Professional Care

The Cardiac Rehabilitation Program staff includes Registered Nurses who specialize in cardiac care. Staff members will work closely with the program's medical director, as well as your primary care physician.

The following services are available:

  • In hospital rehabilitation for patients requiring ongoing medical supervision. 
  • In nursing home rehabilitation focusing on preparing patients for re-entry into their home[HJ1] . 
  • Outpatient Cardiac Rehabilitation for continuing care after patients return home.
