  • Your name and the patient's name and date of birth
  • Your phone number
  • The name of the medication, dose and how often it is taken
  • The name of the pharmacy you use

Please allow 48 hours for your request to reach the pharmacy. Requests received on the weekend will be processed by the end of business, Monday.

Renewal requests for controlled substances (e.g., many ADHD medications) must be called in 1 week in advance, as they are handled only by your child's primary care provider. These prescriptions can only be sent electronically to the Pharmacy.

If we are not seeing your child for regular physicals, we cannot renew prescriptions or process referrals.
