Referral Intake Nurse

$1,600.00 SIGN ON BONUS IF YOU START THIS JOB BY APRIL 28, 2017

The Nurses of At Home Care have been taking care of your neighbors for 30 years. Have you thought about using your nursing knowledge to support them? AHC has an RN opening in our Central Intake Department.

Some of the things you will do in a typical day as an AHC Intake nurse will be:

  • Speak with Physicians, nurses, and community members taking referrals
  • Entering referral information into computer
  • Obtaining physician orders & F2F*
  • Being a resource to the field clinicians

In addition to using ALL your nursing knowledge in this position, you will also need:

  • NYS Nursing License
  • Two years experience in one or more of the following; acute care setting, hospital discharge planning, case management, community nursing, or QI
  • Ability to be flexible & adaptable, detail oriented , and to critically think
  • Ability to successfully function in a fast paced technology driven work environment
  • Excellent computer skills. Able to learn and proficiently use multiple computer programs
  • To enjoy working with people, talking to them, helping them, and listening to them. This is not a solitary position.
  • Patience, lots of patience
  • Excellent customer service & communication skills. Able to effectively communicate with all, with respect and kindness.
  • Availability 8-5 M – F, occasional weekend, holiday, and early evening as needed

Of course, home care is not for every nurse. Being the fastest growing sector of healthcare, it requires that you be highly motivated, self- directing, organized and able to critically think.

Education:

Associates degree, BSN preferred

Experience

Two years experience in one or more of the following; acute care setting, hospital discharge planning, case management, community nursing, or QI

Certifications:

NYS RN License
