Swing Bed Coordinator

Responsible for coordinating the admission of swing bed patients, assessment, welcoming patients to the facility and establishing a Swing Bed Admission Meeting.  Works collaboratively with physicians, nurses, and multidisciplinary team to help patients transition from an acute care setting to a lower level of care.  Acts as a liaison between nursing, rehab and physicians.  Assures appropriate Plan of Care and that it is delivered in a safe environment.  Prepares documents and chairs weekly Interdisciplinary Conference.  Attends daily Interdisciplinary Rounds and participates in patient care discussion.  Works collaboratively with Discharge Planning, Social Work, and Utilization Review RN to assure optimal transition to discharge home setting; offers community resources.  Monitors use of the Swing Beds for compliance with Medicare/Medicaid regulations.  Completes and submits MDS documents per regulatory requirement.  Responsible for data collection and reporting activity.  Participates in Unit Based Practice Councils as indicated.

Monday - Friday, 8:00 am - 4:00 pm

Education:

Associates Degree in Nursing required, bachelors preferred.

Experience

  • Two years nursing experience in an acute care hospital setting, required.

  • Previous experience with Minimum Data Set, preferred.

  • Working knowledge of swing bed programs, discharge planning and utilization review, preferred.

Certifications:

  • Current NYS licensure as a Registered Professional Nurse, required.

  • BLS Certification and ACLS, required.

  • PRI certification, required within six months of hire.

  • NYS Screening certification, required within six months of hire.

Skills:

  • Excellent motor, hand, eye coordination and manual dexterity required.

  • Ability to handle and operate equipment, read, retains and comprehends written and oral medical and technical information.

  • Ability to build rapport quickly, develop trust, sustain working relationships, and represent CRH positively in internal and external meetings is required.

  • Must be proficient with Microsoft Office applications, including but not limited to, Excel, Word, and Power Point.

  • Previous experience with medical records technology, preferred.

  • Strong time-management and organizational skills, required.

  • Effective communication skills and interpersonal skills, required.

Physical Requirements

  • Position requires extensive standing, occasional sitting and routine walking.

  • Ability to lift, possibly heavy lifting in support of patient positioning, instrument and equipment handling.

Travel

  • Travel to Bassett Medical Center and Capital District referring agencies to network as well as screen patients.

  • Travel to Bassett affiliates for networking purposes.
