Case Manager - Social Work

The Case Manager assesses, plans, implements, coordinates, monitors and evaluates options and quality health care services for all patients.  The Case Manager is accountable for collaborating individual specific health care needs in a cost-effective manner.  The Case Manager provides services across a continuum of care.

Key Benefits of this Position:

Monday - Friday, 8:00 am - 4:00 pm

Education:

Master in Social Work, required.

Experience

  • Two years of acute caring nursing experience and/or case management experience, required.

  • Current MDS experience, preferred.

Certifications:

  • NYS licensure as a Licensed Social Worker, required.

  • Case Management certification, Milliman or Interqual certification, and PRI/Screen certifications, preferred.

Skills:

  • Ability to access clinical management of patients through chart review, participating in medical rounds, assessments based on measurable outcomes, protocols, and medical discussions.

  • Ability to apply evidenced based utilization review criteria.

  • Ability to build rapport quickly, develop trust, sustain working relationships, and represent CRH positively in internal and external meetings, required.

  • Must be proficient with Microsoft Office applications, including but not limited to, Excel, Work, and Power Point.

  • Strong time-management and organizational skills, required.

  • Effective communication skills and interpersonal skills, required.

Physical Requirements

  • Walking, standing, and sitting required throughout the shift.

  • Ability to lift, possibly heavy lifting in support of patient positioning, instrument and equipment handling.
