Bassett Healthcare's Department of Pediatrics is made up of health care providers dedicated to the care of infants, children, adolescents and young adults. Through a diversity of medical expertise and sub-specialties, community programs and caring providers, Bassett addresses the overall health and wellness needs of its young patients and their families.

Bassett Healthcare's Treatment of Minors Policy:

The health care providers at Bassett believe that parents provide a most important background for their child's health and wellness, enabling us to give the best possible care to children. It is therefore our policy that a parent or parent's proxy be present whenever we treat minors, except as permitted under NYS law.

If unusual circumstances prevent your being present when we see your child, then we will need a signed copy of either:

• the "Consent by Proxy for Non-urgent Pediatric Care" (authorizing someone else, who accompanies your child, to make health related decisions for you regarding your child's care) or
• the "Pre-authorization to Treat Minors Consent" (if you need to give us a one-time OK to see your unaccompanied child.)
