Bassett Healthcare's Department of Pediatrics is made up of health care providers dedicated to the care of infants, children, adolescents and young adults.
Through a diversity of medical expertise and sub-specialties, community programs and caring providers, Bassett addresses the overall health and wellness needs of its young patients and their families. Bassett Healthcare's Treatment of Minors Policy:
The health care providers at Bassett believe that parents provide a most important background for their child's health and wellness, enabling us to give the best possible care to children. It is therefore our policy that a parent or parent's proxy be present whenever we treat minors, except as permitted under NYS law.
If unusual circumstances prevent your being present when we see your child, then we will need a signed copy of either:
- the Consent by Proxy for Non-urgent Pediatric Care (authorizing someone else, who accompanies your child, to make health related decisions for you regarding your child's care) or
- the Pre-authorization to Treat Minors Consent (if you need to give us a one-time OK to see your unaccompanied child.)
Click on one of the links above and you can print a copy of the form you need, and either fax or mail the completed form to the clinic/health center where your child will be seen.
- For addresses and fax numbers of clinics click here.
We will need the appropriate signed consent form at the clinic where and when your child is being seen without you. These forms are also available at the clinic where your child is being seen. If you have questions about this requirement, please call 607-547-3290 or 1-800-BASSETT.
