How can I get my health records for my own use?

Original health care records belong to the facility generating them. You must sign a “Release of Information” in order to get copies of your records.

How can I obtain a Release of Information?

Call the numbers above to request a release of information for each respective Bassett location. You may mail or fax the release form requesting your information.

How long will it take to get my records?

State law allows us 10 business days to process your request for records. We make every effort to get them to you sooner.

Is there a cost for this?

You may be charged a reasonable cost.

What if my provider or insurance company wants my records?

In general, Bassett Healthcare Network must obtain your authorization to use or disclose protected health information (PHI) unless the Privacy Rule permits or requires use/disclosure. For example, the Privacy Rule explicitly authorizes Bassett Healthcare Network to use and disclose PHI for medical treatment, payment, and health care operations without authorization.

There are three specific situations that require Bassett Healthcare Network to obtain written authorization from a patient (unless classified as an exception or permitted for use/disclosure):

  • the use and disclosure of psychotherapy notes,
  • use of disclosure of PHI for marketing, and
  • any disclosure of PHI which is a sale.
