BENEFLEX - A PROGRAM OF CHOICESAs a Resident you are immediately eligible to join BeneFlex, Bassett's flexible benefits plan. With BeneFlex, residents choose the type of benefit package that best meets the needs of their family situation and life-style.
MEDICAL BENEFITS
Our flexible benefits program offers you and your dependents three medical options for comprehensive medical coverage. Covered services include physician visits, preventive care, hospitalization, surgery, emergency care, and more.
Bassett Plus HMO $15 (Bassett Network) includes a $15.00 physician visit co-pay, a $50.00 emergency room co-pay, NO well visit co-pay, NO hospital inpatient deductible, and NO outpatient surgery deductible.
Bassett Plus HMO $20 (Bassett Network) includes a $20.00 physician visit co-pay, a $50.00 emergency co-pay, NO well visit co-pay, a $500.00 hospital inpatient deductible ($1500 annual max), and a $100.00 outpatient surgery deductible.
Bassett Plus HMO $20 Lower Cost Option (Bassett Network) includes a $20.00 physician visit co-pay, a $50.00 emergency co-pay (and $50.00 Ambulance co-pay), a $20.00 well visit co-pay, $500.00 hospital inpatient deductible, a $500.00 inpatient rehabilitation deductible, and a 20% co-insurance or $200 outpatient surgery co-pay.
Bassett Plus PPO. In-Network (15 for routine physical, no routine eye or hearing exam coverage). Out-of-Network available with 70% coverage (for covered services) after deductibles are met.
PHARMACY PLAN
Bassett's Pharmacy Plan pays 60% of the cost of prescription drugs or refills. There is an additional 10% employee discount at the Bassett Outpatient Pharmacy.
EYEWEAR PLAN
Residents and their dependents receive an allowance towards the purchase of lenses or contact lenses, and frames at the Bassett Eyewear Center.
DENTAL BENEFITS
We offer Residents and their dependents two dental plans. One provides coverage for preventive and minor restorative care (fillings, extractions, and root canals) and the other plan provides additional coverage for major restorative work (crowns, bridges, and dentures) and orthodontics. The Comprehensive Plan has a calendar year maximum of $1500 not including orthodontia. Comprehensive covers 100% of two routine visits per year (check-up, cleaning, x-rays). It also covers 80% of Basic care (fillings, root canals, extractions, oral surgery, periodontal). It provides coverage for major dental work such as prosthestics, crowns, bridges, and dentures at 60%. Orthodontia coverage at 50% to a maximum of $1500 (lifetime maximum). Preventive covers 100% of two routine visits per year (check-up, cleaning, x-rays). It covers 60% of basic care (fillings, root canals, extractions, oral surgery, periodontal) after an annual deductible.
GROUP LIFE INSURANCE AND AD&D
We offer basic and supplemental term life insurance. Basic Life Insurance - we pay the full cost of coverage for one times your annual base pay. Supplemental Term Life Insurance - you may elect an additional amount of life insurance from 1 times up to 3 times your annual base pay at discounted group rates.
AD&D insurance provides benefits for you and your dependents should one of you die or be severely injured in an accident. We pay the full cost to provide coverage of one times your annual base pay. Under the AD&D plan you can purchase additional coverage up to 3 times your annual base pay.
SHORT-TERM DISABILITY BENEFITS
We provide coverage for those times when you are unable to work due to an illness, injury, or pregnancy. This plan provides a weekly source of income beginning on the eighth day of absence and may continue for up to 26 weeks.
LONG-TERM DISABILITY BENEFITS
Long term Disability provides income replacement if you become ill or injured and are unable to work beyond a six-month total disability period. Benefits are payable for as long as your doctor and the plan administrator agree that you are unable to work. This benefit is provided to Residents at no cost.
FLEXIBLE SPENDING ACCOUNTSHealth Care Spending Account - you may direct up to $5,000 of your pre-tax pay into this account each calendar year. You can use this account to reimburse yourself for expenses such as health plan deductibles, coinsurance amounts, certain over-the-counter drugs, and other expenses not covered by the medical, dental, and pharmacy plans. Dependent Care Spending Account - This plan allows you to pay for certain child and elder care expenses with tax-free dollars. You may direct up to $5,000 of your pre-tax pay into this account and use it for qualified dependent care expenses.
VOLUNTARY BENEFITS THROUGH PAYROLL DEDUCTIONS
There are several voluntary benefits that are also made available to Residents through the convenience of payroll deduction. For example, through a single vendor employees can sign up for auto insurance, homeowners, and renters insurance. Voluntary Short Term Disability and Cancer Care coverage (AFLAC) are also available through payroll deduction along with Supplemental Life Insurance coverage through Security Mutual, Voluntary Dependent Life Insurance through Allstate, and coverage for certain legal expenses through Prepaid Legal, Inc.
TRAVEL LIFE INSURANCEThe policy covers residents for $100,000 when traveling on business.
PROFESSIONAL LIABILITY INSURANCEBassett provides full coverage for hospital professional liability.
WORKER'S COMPENSATIONResidents are covered under the New York State Worker's Compensation plan.
PERSONAL INSURANCE PLANPurchase of supplemental permanent universal life insurance through Security Life Insurance Company is available twice each year. The coverage provides a death benefit, a loan feature and builds cash value based on current interest rates. Residents and their family members are eligible and premiums are paid by payroll deduction.
Auto and Homeowners/Renters Insurance are underwritten by New York Central Mutual at discounted rates and are available to residents and paid through payroll deduction.
TAX-DEFERRED RETIREMENT SAVINGSThis plan is offered in the form of a voluntary Tax Sheltered Annuity (TSA), also known as a 403(b), to the employees of Bassett. This plan allows you to deduct money from your pay before taxes are withheld, in order to save for retirement.
VACATIONThe vacation rate for all residents is three periods of seven consecutive days each, per academic year.
HOLIDAYSBassett observes six calendar holidays per year, which will have a variable effect upon the house staff depending on service requirements. Up to four additional elective holidays will be allowed on an informal basis.
CAREER DAYSUp to four days in the resident's final year may be allowed informally as career leave days after advance arrangements have been made with the Program Director and Attending in charge of the resident's service for the purpose of interviewing for fellowship or practice positions.
EDUCATIONAL ALLOWANCEBassett provides seven days per academic year for PGY-2 - 5 residents for educational leave including travel. PGY-1 residents will receive an educational allowance of $500 per academic year to purchase medical textbooks, medical journal subscriptions, medical software, medical society dues, PDA software and conference tuition and travel. PGY-2 and PGY-3 residents will be allowed up to $1,250 per academic year for educational expenses, while PGY-4 and PGY-5 residents are allowed $1,750 per academic year for educational expenses. PGY-2 - 5 may use up to $1,000 to purchase a computer. This is a one-time benefit. All PGY levels may use up to $200 toward the purchase of an IPOD. All educational travel must be of value and related to the training program and approved by the appropriate Program Director. Other educational travel may be approved on an individual basis by the appropriate Program Director or Director of Medical Education.
LICENSUREBassett pays initial and subsequent New York State licensing fees for all eligible PGY 2 house officers and beyond during their training at Bassett and requires that house officers be licensed by October of their PGY 2 year.
UNIFORMSUniforms and laundering of uniforms for house officers are provided through a rental service paid for by Bassett.
ON-CALL SNACK ALLOWANCEAn on-call snack allowance and a stocked snack refrigerator is provided for residents on nighttime duty.
MOVING ALLOWANCEResidents moving to Cooperstown for the first time will receive a gross payroll check in the amount of $650. Individual check amounts will vary based on tax withholdings.
LEATHERSTOCKING REGION FEDERAL CREDIT UNIONThe Credit Union is a member owned, non-profit financial institution. It offers many services including payroll deduction saving plan, Christmas Club, and loans (car, mortgage, home equity, and personal) at reasonable rates. Checking accounts, direct deposit, and credit cards are also available.
TEXT PROCESSING SERVICEText processing services are available to residents, at no cost, for clinical or educational use.
HOUSE STAFF LOUNGEA House Staff Lounge including cable television, video cassette recorder and compact stereo is available for use 24 hours per day and includes a stocked refrigerator for residents on call.
COMPUTERSComputers are equipped with Windows, word processing, database, spreadsheet and presentation graphics software, as well as Internet access, are available for use by residents.
PHOTOCOPYINGUnlimited photocopying for professional needs is available at no charge.
PRIMARY CARE DEBT FORGIVENESSResidents in the Columbia-Bassett Primary Care Internal Medicine Program who are committed to practicing primary care are eligible for up to $10,000 per year to be used towards payment of student loans. Funds are awarded after the successful completion of each post-graduate year to a maximum of $30,000.
Residents are also eligible to participate in a Primary Care Loan Forgiveness Program administered by New York State.
PDAS
Personal Digital Assistants, such as Handspring Visors, are used in all programs and are provided on a shared cost basis with residents. Residents are wholly responsible for replacement of lost or stolen PDAS.
