  • Clinical Pharmacist Care

    A clinical consultative program specifically designed to reduce the risk of fall and injury in the home.

    Patient Eligibility:

    • Patient with history of falls
    • Patient prescribed 10 or more medications
    • Consultative support requested by patient physician

    A clinical pharmacist (PharmD) will review baseline clinical information, to include lab data and medication profile, for purposes of identifying potential for adverse reaction and interaction that may increase the risk of fall. Based upon the review, a consultative report is forwarded to the physician
