Office visits are by appointment only. All patients under the age of 18 must be accompanied by a parent or legal guardian.

We make every effort to see our patients on time. To help us maintain an efficient schedule, please arrive 10-15 minutes before your scheduled appointment to allow for registration. If you arrive more than 15 minutes late for a sick visit, you will be asked to wait for the next available appointment slot. If you arrive more than 15 minutes late for a well child check or follow-up, you may be asked to reschedule the appointment. Please call 24 hours in advance if you cannot keep a scheduled appointment.

Well Visits
Regular well child checks are scheduled at 2 weeks, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months and 2 years. After age 2, well child checks happen once per year. Immunizations are given at many of these visits, and the professionals at Child & Adolescent Healthcare Associates are strong proponents of complete, on time immunization.

Our busy season for physicals/well child checks is summer and early fall. Please call several weeks in advance if you need a camp/school/college form completed by a specific deadline.

Please note that if we do not see your child for regular physical exams, we will not be able to renew prescriptions or process referrals.

Payments and Insurance
Please bring proof of insurance to every visit. Copayments and patient responsibility percentages are due at the time of service from the person accompanying the patient. If there is no proof of insurance, payment in full is expected.
