Diabetes Management
Many patients with diabetes lead a normal, healthy life. How? By managing their condition successfully through education and lifestyle changes.
You can learn to do the same.
Fox's Diabetes Management Program-which follows the national standards endorsed by the American Diabetes Association-is open to everyone with diabetes, whether or not they're insulin-dependent. A certified diabetes educator coordinates the program with a registered dietitian and registered nurse providing both group and individual sessions for each patient.
In two group sessions, you'll learn about the relationship of lifestyle to blood glucose levels, options for improving control of diabetes, preventing complications and much more.
Individual counseling sessions will provide each patient with a comprehensive assessment, a nutrition care plan, glucometer instruction and insulin administration as indicated by their Health Care Provider.
