Our mission is to help patients who need to use additional services or follow special instructions after they leave. Your discharge planning team-your doctor, your nurses, social workers and other healthcare professionals-can instruct you in taking medicine, changing dressings and using medical equipment. If necessary, we help you arrange for life skills assistance or even find a special nursing facility that's right for you.

You play a critical role on this team. The more you tell us about your health, home, family, interests and insurance coverage, the better we can determine-together-what you'll need when you leave the hospital.
